PEN-221 (PEN221) is a novel and potent Somatostatin receptor 2 (SSTR2)-targeting conjugate consisting of microtubule-targeting agent DM1 linked to the C-terminal side chain of Tyr3-octreotate. As a peptide-drug conjugate (PDC), PEN-221 demonstrates in vitro activity which is both potent (IC50 = 10 nM) and receptor-dependent (IC50 shifts 90-fold upon receptor blockade). PEN-221 targets high levels of DM1 to SSTR2-expressing xenograft tumors, which has led to tumor regressions in several SSTR2-expressing xenograft mouse models. The safety and efficacy of PEN-221 is currently under evaluation in human clinical trials. PEN-221 is a miniature drug conjugate consisting of a peptide ligand, that is highly selective in targeting SSTR2, joined through a cleavable linker to the potent cytotoxic payload DM1. SSTR2 is overexpressed on the cell surface of a range of solid tumors including neuroendocrine tumors and small cell lung cancers. In non-clinical experiments, PEN-221 binds with high affinity and selectivity to SSTR2. On binding, PEN-221 triggers SSTR2 internalization resulting in the accumulation of the DM1 payload in tumor cells followed by cell cycle arrest and apoptosis.

Nendratareotide uzatansine is a peptide-drug conjugate composed of a somatostatin peptide analog and the cytotoxic metansine. Nendratareotide uzatansine is a miniaturized drug conjugate consisting of a somatostatin peptide analog targeting somatostatin receptor 2 (SSTR2) and conjugated to the microtubule-binding cytotoxic metansine compound DM1 (metansine) via a cleavable linker, exhibiting potential antitumor activity. Upon administration, the peptide ligand portion of Nendratareotide uzatansine targets and binds to SSTR2 overexpressed in certain tumor cell types. This binding stimulates SSTR2-mediated drug endocytosis; after endocytosis, the DM1 portion is released and binds to tubulin, thereby disrupting microtubule assembly/depolymerization dynamics. This inhibits both cell division and the proliferation of SSTR2-expressing cancer cells. Compared to antibody-drug conjugates (ADCs), miniaturized drug conjugates are smaller in size and more easily penetrate and distribute within dense tumor tissue.
