Sudoterb (formerly also known as LL3858; LL-3858) is a novel and potent drug candidate with the potential for treating Tuberculosis (TB). TB drug research and development efforts have resurged in the past 10 years to meet urgent medical needs, but enormous challenges remain. These urgent needs are largely driven by the current long and arduous multidrug regimens, which have significant safety, tolerability and compliance issues; rising and disturbing rates of multidrug- and extensively drug-resistant TB; the existence of approximately 2 billion individuals already latently infected with Mycobacterium tuberculosis, the causative pathogen of TB; and a global TB-HIV co-epidemic.

The recently synthesized pyrrole Sudoterb (LL3858) (Fig. 3) showed higher bactericidal activity compared to isoniazid. Sudoterb is active against M. tuberculosis strains that are resistant to available anti-TB drugs [1].
Basic biology information [3] Drug target/mechanism: Not known; Sudoterb (LL3858) is a pyrrole derivative In-vitro potency against MTB: M. tuberculosis MIC: 0.12 0.025 mg/ml. Other in-vitro activity: The compound demonstrates in vitro synergy with rifampin. It is bactericidal in 12 days with concentration-dependent killing. Other pyrroles have shown activity against TB. |
| ln Vivo |
The effectiveness of Sudoterb (LL3858) (12.5 mg/kg and 25 mg/kg, sidewall, 4 and 8 weeks) against M. mice infected with TB [2]. The mouse oral drug sudoterb has an LD50 of 700 mg/kg [2].
In-vivo efficacy in animal model: In mouse models of 12 weeks duration 12.5 mg/kg Sudoterb (LL3858) showed good efficacy and complete clearance from lung and spleen not seen with the other compounds tested. No relapse was observed up to 2 months following the final dose.[3] Efficacy in humans Web releases (for example Media coverage summary 7 2004) indicate that clinical trials may begin but the Lupin website does not post any further information on this compound [3]. |
| Enzyme Assay |
Preliminary in vitro antitubercular screening [1]
MIC of the selected compounds 3a–c, 3f–k, 4a–d, 5, 6 and 7 were subjected to antimycobacterial evaluation against M. tuberculosis H37Ra 7131 strain in liquid medium by the broth dilution method according to modified method of Heifets. |
| ADME/Pharmacokinetics |
In mice, the pharmacokinetic properties of this drug were superior to those of isoniazid, with better half-life, Cmax, and AUC. [3]

| Additional Infomation |
The discovery and development of new drugs for tuberculosis (TB) has been revitalized in the past decade. TB is a spreading and deadly bacterial infection that causes more than 2 million deaths annually and has attracted increasing attention in both developing and developed countries. This article reviews drugs currently under clinical evaluation for the treatment of uncomplicated and drug-resistant TB and updates the literature on five new drugs that have entered clinical trials in the past four years. [2]
This article describes the synthesis of several novel 3,4-disubstituted thiazolyl methylene isoniazid derivatives 3a-k, 2-substituted thiazolyl isoniazid derivatives 4a-d, and pyrrolidinyl isoniazid derivatives 5, 6, and 7. The in vitro anti-tuberculosis activity of the resulting compounds was evaluated. The minimum inhibitory concentration (MIC) of compound 3 g at a concentration of 9.77 μg/mL showed in vitro activity comparable to isoniazid against Mycobacterium tuberculosis strain H37Ra 7131. Solubility in Formulation 1: ≥ 2.5 mg/mL (4.81 mM) (saturation unknown) in 10% DMSO + 90% Corn Oil (add these co-solvents sequentially from left to right, and one by one), Clear solution.
For example, if 1 mL of working solution is to be prepared, you can add 100 μL of DMSO stock solution (25.0 mg/mL) to 900 μL of corn oil and mix well.  (Please use freshly prepared in vivo formulations for optimal results.) |
| Preparing Stock Solutions | 1 mg | 5 mg | 10 mg | |
| 1 mM | 1.9247 mL | 9.6233 mL | 19.2467 mL | |
| 5 mM | 0.3849 mL | 1.9247 mL | 3.8493 mL | |
| 10 mM | 0.1925 mL | 0.9623 mL | 1.9247 mL |
*Note: Please select an appropriate solvent for the preparation of stock solution based on your experiment needs. For most products, DMSO can be used for preparing stock solutions (e.g. 5 mM, 10 mM, or 20 mM concentration); some products with high aqueous solubility may be dissolved in water directly. Solubility information is available at the above Solubility Data section. Once the stock solution is prepared, aliquot it to routine usage volumes and store at -20°C or -80°C. Avoid repeated freeze and thaw cycles.

Working concentration: mg/mL;
Method for preparing DMSO stock solution: mg drug pre-dissolved in μL DMSO (stock solution concentration mg/mL). Please contact us first if the concentration exceeds the DMSO solubility of the batch of drug.
Method for preparing in vivo formulation::Take μL DMSO stock solution, next add μL PEG300, mix and clarify, next addμL Tween 80, mix and clarify, next add μL ddH2O,mix and clarify.
(1) Please be sure that the solution is clear before the addition of next solvent. Dissolution methods like vortex, ultrasound or warming and heat may be used to aid dissolving.
(2) Be sure to add the solvent(s) in order.
