Nivolumab (BMS-936558; ONO-4538; MDX-1106; Opdivo) is a programmed death receptor-1 (PD-1) blocking antibody, known also as therapeutic antibody of PD-1, that has been approved for use to treat advanced (metastatic) non-small cell lung cancer. Nivolumab is used as a first line drug for the treatment of inoperable or metastatic melanoma in combination with ipilimumab if the cancer does not have a mutation in BRAF, as a second-line treatment following treatment with ipilimumab and if the cancer has a mutation in BRAF, with a BRAF inhibitor, as a second-line treatment for squamous non-small cell lung cancer, and as a second-line treatment for renal cell carcinoma. Nivolumab has recently been approved for small cell lung cancer.

Nivolumab does not attach to the ancestral CHO cell line, however it does bind with an EC50 of 1.66 nM to PD-1-expressing CHO cells. Nivolumab has an EC50 of 0.64 nM for binding to PD-1 on activated T cells. With IC50 values of 2.52 and 2.59 nM, respectively, nivolumab also inhibits the interaction between PD-1 and its ligands, PD-L1 and PD-L2. When T cell receptor stimulators are present, nivolumab (1.5 ng/mL) increases T cell responses [1].

In cynomolgus monkeys, nivolumab (10 and 50 mg/kg, intravenously) was well tolerated. Serum chemistry changes at week 13 in female mice treated with 50 mg/kg were restricted to a reversible 28% drop in T3. TSH and T4 levels were constant. Men administered with 50 mg/kg showed no change in T3, T4, or TSH levels. Between 10 and 50 mg/kg, nivolumab exposure rose in a roughly dose-proportionate way; no discernible sex differences were seen [1].
